Hi Iā€™m Shannon Wood,

I am a Gerontologist with over 25 years of experience working with seniors as a social worker and care manager.

The focus of my practice is always to honor the unique experience and perspective every person brings to the journey of life.

My credentials include a Bachelors Degree from the University of Oklahoma and a Masters Degree in Gerontology from the University of Central Oklahoma. I have worked in the aging field since 1990, and more specifically within the Denver community since 1998. I have extensive experience as a social worker, care manager and as a community relations representative in home care.

At Beacon Care Management we have an in-depth understanding of Geropsychology, end of life issues, family systems, and elder wellness.

Our passion is helping you find the best care solution for your loved ones. We look forward to meeting you.

What are the benefits of hiring a care manager?

Cost containment

With the involvement of a private care manager and coordination of care, duplication of services are eliminated. Unnecessary hospitalizations and inappropriate placements are avoided.

Decreased Social Isolation

When a person ages and struggles with failing health, their world becomes very small. A care manager can link the client with creative ways to remain socially engaged.

Coordination of medical care

A care manager can attend doctors appointments, making sure orders are understood and followed, leading to better compliance, and alleviation of unnecessary hospital stays.

Arranging and monitoring home services

A care manager can determine which at home services are appropriate. This can include personal care, chore services, medical services, and safety modifications. Once these services are in place, the care manager will provide ongoing monitoring to ensure the client is receiving the best care to remain as independent and safe as possible.

Caregiver Support

A care manager can step in to make needed decisions based on comprehensive assessments. Family members will be less likely to make hasty and emotional decisions regarding care. Caregiver stress will decrease with the added support.

Monitoring of care in a facility

Whether it is coordinating a move to a higher level of care/facility, or simply monitoring the care provided, the care manager is invaluable. A care manager can facilitate communication between facility staff and families, as well as medical providers.
